Supporting families with disabled children in Waltham Forest
WFPF works closely with the borough short break co-coordinator to identify short break services and fun activities for disabled children. Short break services are after school clubs, weekend clubs, overnight stays, youth groups and holiday schemes provided by Waltham Forest to disabled children. To apply for a short break families need to either complete a self referral form or have a social worker assessment. For more information on how to apply for a short break please contact Tony Connole, Short Breaks Coordinator, LBWF, Juniper House, 221 Hoe Street, Walthamstow E17 9PH. E-mail: firstname.lastname@example.org. Tel: 020 8496 3515.
